Credentialing
If you are interested in operating at Charlestown Private Hospital, please send an inquiry to our General Manager/Director of Nursing Connie at C.McLachlan@charlestownprivate.com.au and she can arrange a tour of our facility.
We will send you a credentialing application pack by email, and request your:
- AHPRA Registration
- Indemnity insurance Certificate
- Current College CPD
- CV
- Copy of relevant Degrees and Fellowship
- Photo ID either Drivers licence or Passport
- Current NSW Working with Children’s Check
- Three references.
Our General Manager/Director of Nursing will check your references and present all documents to the Charlestown Private Medical Advisory Committee.
Following approval at the next Medical Advisory Committee meeting, full privileges can be granted for a 3-year term.
Hospital by-laws
All Nexus Hospitals operate under by-laws approved by the Nexus Hospitals Board. These by-laws empower medical staff, giving assurance of quality hospital medical care.
The by-laws outline the management policies approved by the Board, and document:
- the Terms of Reference for the Charlestown Private Hospital Medical Advisory Committee
- the process of accrediting practitioners at Charlestown Private Hospital and defining their scope of practice
- the obligations of accredited practitioners in relation to the safety and quality of care patients receive, including patient privacy, open disclosure, infection control and reporting, and medical research
- the expected conduct of accredited practitioners at Charlestown Private Hospital.
